Champions Parisi Speed School Unlimited Membership Contract

• This is a commitment to make 3 consecutive monthly payments for Unlimited Parisi group sessions per month.
• I understand that I must give a 30-day notice to cancel. If my notice to cancel is within 30 days of the next payment, I will be charged one more time.
• I understand when returning to Champions after a cancellation, I will be charged $25 to re-enroll.
• Member understands that signing up for each Speed School class is mandatory.
• Champions will release me from this contract if 1) The member moves 30+ miles from the facility, or 2) Proof is provided that the member is medically unable to participate.
