Terms and conditions for Parisi 6x Month Open House Special

This is a commitment to make 3 consecutive monthly payments.
I understand that after my 3 consecutive payments, I can cancel as long as I give a 30 day cancellation notice. If my notice is withing 30 days of my next payment, I understand I will be charged one more payment.
Member understands that signing up for each Speed School class is mandatory.
Champions will release me from this contract if 1) The member moves 30+ miles from the facility, or 2) Proof is provided that the member is medically unable to participate.
