Champions Community Heroes Membership Contract

-This is a commitment to make 6 consecutive monthly payments.

- I understand that I am getting a discounted rate for the 6-month commitment.

- This membership is open-ended, and my payments will continue after 6 months until I cancel. It is my responsibility to cancel my membership if I so choose after the initial 6-month contract.

- Champions will release me from this contract if:
1) I move 30+ miles from the facility.
2) I provide proof that I am medically unable to perform the TFW or HI-LI workouts.
